Attachments don't stay attached on first try

Anyone else experiencing thing?  Or happen to have a fix?  I'll attach something, give it time to load and complete, and then when I refresh the task (or assign it to someone) the attachment isn't there and I have to redo it.  Doesn't matter if it's an image, Word doc, PDF.  Really annoying and definitely not making work more efficient having to do things multiple times.
