How to determine if an attachment was sent internally or externally?

I am asking because sometimes the recipient does not see the attachments I send.

Welcome to community!

For JSM, attachments in internal comments has a lock icon in the issue view section of the attachments. The ones that are customer facing do not have that lock. Hope this helps!
