Adding Instructional Text/Template to Kanban Ticket in Description Box

Is there a way in classic JIRA to display instructional text or a template that appears in the Description field for every new ticket that is created?

 

Ie. 

Name:

DOB:

Gender:

I use the server version of this product. It should be able to do what you want.
