We couldn’t save your approval response

Hello team, 

Jira service management issue: We couldn’t save your approval response while approving the change request. 

1) I"m using the default Change management workflow as below. 

2) Change approve automation which is working fine as per the below screenshot. 

3) Error while approving a request. 

Please help to fix this error.
