With a VSB import the file must include a header row that list the fields for the data in the test of the file.
Optionally it can also include a field/column that identified the project in Jira where the issues will be created.
There are different methods to access the CSV import functionality which can result in showing different prompts. Exactly what steps did you execute to get to the CSV import functionality?
