can't provide feedback, something when wrong

When I have a 'ticket' open and I click on "give feedback" I get:

that one occurs on page load.  this one occurs upon clicking "give feedback":

Hi Mike,

Sorry to hear about this problem.  It looks like there is a problem in regards to an add-on that your Jira Cloud seems to be running, specifically the Connector for Salesforce & Jira.

You could either create a support case for this directly the plugin vendor or our Cloud support team.  However it looks like your account is not the site-admin of the Jira Cloud site, hence you won't be able to do this yourself directly.  

I'd recommend that you reach out to one of the site-admins or billing contacts for your Atlassian Cloud site and ask that they raise a support request on your behalf.  They can do this by going to https://support.atlassian.com/contact

However I fear if your account tries this, you would just get redirected back here to Community again.

Let me know if you run into any problems with either approach here.
