Transition screen fails to load intermittently

Hello,

We have had users report recently that they are noticing that when performing a status transition the transition screen will fail to load. This appears to be uncommon, but I have gotten enough reports now that I feel this is ether a config issue or a Jira bug.

Has anyone else experienced this issue before? If so, were you able to root cause/solve?

I have never seen this before.

This could only be the case if there is separate transition to the same status without a screen.
