Is it possible to synchronize single issues between JIRA Cloud and JIRA Server instances?

Yes, you can definitely synchronize Jira Server and Cloud.
You will have to use an app for that. We are the developers of the Exalate Issue Sync app.
You'll be able to synchronize any issue data of one of your instances to the other.
This will happen in real-time. That way, you'll always be in the loop on progress status, comments, attachments and even work-logs or change history.

as long as your internal Jira can access the internet, you don't need to change any firewall settings - that's at least true for Exalate and Backbone Issue Sync. You can also check out the corresponding information in their docs:

Does the server and cloud instance have to be exactly the same? (i.e user tier, user name, version, permission, etc)

No, this is not necessary. Server and Cloud instances could be have completely different user tiers, permissions and so on. If you are interested on where to start with Backbone, here is the link to the documentation.

You can use existing solution to synchronize multiple Jira instances like IssueSYNC.
It works for:
- cloud
- server
- data center deployments and supports different Jira version 6, 7, 8 and Jira editions

You setup whole integration via UI and you can let your project administrator's to setup/tune own synchronization for remote Jira connections prepared by sys admin before.

Instead of synchronizing the issues you could also create Applinks between the Cloud and Server instances and then use the JIRA Issue Linking feature. Then you could at-least see then links between your issues.
