A approval flow was designed in work management where we identified the need to upgrade licenses to access the Approvals' functionality.
We currently don't have any premium licenses, but the decision to make the purchase depends on whether premium licenses are only required for the flow designers managing the project, or if they are also needed for anyone in the organization who needs to interact to approve or deny a task.
The question in question is summarized as:
Are premium licenses required for anyone who needs to approve or deny, or just for those who manage the workflow?
I need to estimate a budget and I can't find an answer.

Approving/denying users is done by an Organization Admin above the product. The Organization admins do not have to occupy a product license (although it is beneficial sometimes). This is independent of the plan (Free/Standard/Premium/Enterprise).

Can you give a bit more context? What products are you evaluating? You buy premium licenses either for all of your users or none of them. A premium license for Jira, Confluence, ect, unlocks additional features for most of your users. Some features would only be available on the administration panel on the back end.
