Using automation, how to copy a proforma form from the parent issue to subtasks

Using automation for Jira Service Management, how can I copy a proforma form from the parent task to subtasks.

The form shows on the parent task, the subtasks get created but the form does not copy. I previously setup each subtask in a different branch.

The error that I get is No forms attached.

You need to change the scope of your automation rule to something similar to this one:

I tested it in the format I am sending you and it worked fine. If you succeed like this, the form will be properly copied.
