Problems creating a URL to an Issue Collector

I'd like to create my own button that links directly to an issue collector. When I use the following url:

https://dsa-projects.atlassian.net/rest/collectors/1.0/template/form/cc5edc64

I get a blank screen. I checked the admin console for the issue collector and the collector in question is not disabled.

Can anyone help?
