Problem with JIRA Issue collector form using a custom trigger

I created an issue collector and the form renders fine when I use one of the 3 default triggers but when I use a custom trigger, 2 forms show up on the pop up.  There is a half of a form above and a half of a form below.  it is two forms and not one that is skewed because I submitted both with different outcomes.  Attached image is what it looks like.
