Do I need to pay for Jira Service Desk Management if I already have paid Jira Software?
Both are different setups.
If already 3 agents there in JSM. for adding fourth agent will cost you.
External user license is free. there is no limit for users.
The short answer would be "Yes".
The JIRA platform has 3 applications that are licensed separately.

You can also get different tiers of users for each product so you don't have to match the tier of Software with the number of agents.
As @Kagithala Babu Anvesh mentions, customers (so the users that just create requests) do not require a license.
