Microsoft Word Document with Check Boxes

I have a Microsoft Word document (Template I created) and used the developer tab to add check boxes (under control) in an effort to simplify the document for the person filling it out.  It's a template made to collect the most important information needed for projects/demands/enhancements in Jira.  Once the document is filled out I upload it to Jira. However, the check boxes disappear when to click on the document in Jira.

@Laurie Thelwell Welcome. You might want to, once the doc is finished, export as pdf and actually attach the pdf instead to the jira issue?

@Laurie Thelwell If you follow this approach, the Better PDF Automation app can reduce the work required!

As part of a standard Jira Cloud automation rule, this app can generate a PDF and attach it to an issue. It sounds that this is what you need.
