JSD multi-part forms?

I have JSD Data Centre. I need to create a multi-part form for customers to fill in. Ie a standard incident form, plus a customer needs to enter their address history.

As you can see, their address history could to from 1 to unlimited. So how do I do that? is there an extension I can get?

I've got the Dynamic Forms plugin but that's only useful for 1 set of address.

The only other way I can think of is getting the customer to fill out the form, and then fill out another form for address history. Then I'll need to link them up in the backend.

Hi There

there is a plugin called STAGIL tables which supports what you are looking for

You may create a one column (address) table and user may add as many entries as desired
