Can anyone help me with the JQl statement for identifying on what and all issues, the description has been modified or changed ?

Hello @Haran Kumar 

It's not possible out of the box. Because description field doesn't support the "Changed" operator

You might have to use custom scripts from paid plugins to achieve this feature.
