There's a single user whose account is reset each time the user logs into JIRA. By reset - I mean any changes to her name are lost and the email field is cleared.
This causes the user to stop receiving email notifications.
Any ideas on why this may happen?
Checked with IT and the user's email address was not populated in their AD account. Adding her email address fixed the issue.

Hey Katherine! Have you checked the logs for any relevant message related to this? Normally, JIRA logs changes to an user to the "atlassian-jira.log". This can be caused by a wide variety of variables. The most important thing would be to know how you have configured the LDAP server and also what is its type. Another thing I want to confirm is if you are making those changes to the LDAP server itself or through JIRA? And are you sure this is happening only to this user? Cheers!
