How to measure Escaping Defects?

We have 3 environment(QA, Staging, Production), now when ever a bug is logged to jira on Qa environment and that bug slips to production also due to some reasons. So how to track these issue on dashboard reporting part?

So basically I have few question related to this:

1. Escaping Defects from Dev to QA, QA to Staging, Staging to Production?
2. Dashboard of Aging Defects?
3. Average time taken for Defect Resolution?
4. Reopen Rate?
