How to generate a jira ticket from Forms of Confluence

I have created a Forms in confluence and getting that requests via email but i want those ticket to be created in Jira tickets in our particular board

Are you referring to this app? https://marketplace.atlassian.com/apps/261/forms-for-confluence-capture-feedback-with-surveys-polls?tab=overview&hosting=cloud 

As far as I know, Forms for Confluence doesn't support Jira issue creation through form submission yet, but please check with the vendor directly for more details :)
