Would it be possible for someone to guide me in the right direction with regards to creating a custom form?
What I mean by that is, when, for argument sake, you move a ticket from one "swim-lane", to another, a popup normally appear, ie: "Start Work", or something like that. I want to create a custom form, that would pop up as soon as a ticket gets dragged into a specific "swim-lane", so that I can capture missing fields, that would be required for a ServiceNow Change Request.
"Forms ", I think it's called transition screens.

You can't move issues between swim-lanes. I think you mean "columns".
Have a look at the board configuration, checking what status are in which columns. Note down where you want the screen to appear for each set, so that you know what changes from each status to each status should pop up.
Go to Admin -> Screens, and create a new screen for each different one you want to use.
Edit the workflow(s) for the project(s) represented on the board. Look at each transition between two columns that needs a screen. Edit the transition screen, selecting the one you want from your new list of screens.
