Go to the right side of your screen then click on the icon I highlighted. Once the list displays on your screen, uncheck the issue type box

Also how do you get to this screen? This doesn't seem like a familiar Jira view

Once you click on issues you should see a screen similar to the one I sent you earlier.

No, the check box is not clickable and if you hover there you have the option to add a sub task
