Hope you are doing well.
I have assigned Affect version/s to the project's screen schemes. (default and Bug) (check screenshot)
In addition, when I am checking out, issue layout, I can see Affect version/s. (check screenshot). I have also associated affect versions to my project's screens from view field configuration.
when I am trying to create an Issue, I can't find the affect version/s. When I look for the Affect version/s using Where is my field, it gives below error:
"The 'Affects Version/s' field is not present on the form you are viewing"
Any thoughts?

The Affect version/s field is related to the current versions that you have configured for your project.
Can you confirm if you are using JIRA Server or Cloud? If you are on Cloud, can you confirm if you are using a classic or a next-gen project?
I would check the following steps on a classic project:
1 - Navigate to the project > Project Settings > Screens > Double check if you have configured the create issue screen with the Affects version/s in the relevant issue types
2 - Navigate to the project > Project Settings > Fields > Actions > Edit fields > Check if the field is properly added to the field configuration scheme of the project and check in the Actions column if the field is not Hidden
3 - Navigate to your project board and check if you have properly configured a valid version in your project
For a Next-gen project, please make sure you have turned-on the Releases feature and Double check if you have configured the create issue screen with the Affects version/s in the relevant issue types
