Adding a description field to my workflow.

Hello everybody, 

I'm fairly new to Jira, but I'm slowly getting use to it, although I seem to be stuck on something. 

I currently have a workflow which I made, but I want to add a description field to that specific workflow. 

For example, I have a section on my workflow which rejects a report. So what I want is, if the user who sees the report clicks on rejected, I would like a field to pop up (a mandatory field, if possible) asking the user to write a description as to why the report had being rejected. 

Is this possible? Any help would be greatly appreciated.

This widget could not be displayed.

Should just need to assign a custom screen to the "Rejected" transition in the workflow, with a mandatory custom field.
