Only users with “Issue Create” permissions can submit this form. Your project is not currently set up to enable “Any logged in user”, which may prevent other users from submitting the form. If you wish to enable this, please contact your administrator.

If it's a team-based project, what are the project permissions set to? Open, Limited or Private?
If it's set to Limited or Private, have you added the relevant people in order to allow them to create tickets? Changing it to Open should obviously also fix this issue.

Youre correct, its a team-based project and its under private; the user has been added as member yet when he choose to create a form, it shows contact your administrator...

I was reading about it and it looks like the assigned user needs to have Jira Software licence to create a form, is this accurate?
