Adding a new field to an existing form

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!

I have selected a Template form in Jira Work Management. However I am unable to create a new field and add the field to my form.

I don't have the option of creating a new form. Only existing fields can be selected when using the template.

Can anyone help?

Here are two links that may be of use to you depending on how you want to do this.  First, the general directions are here:

If you want to create a custom event, you'll create the event, add it to the notification scheme, then add it to the workflow:
https://confluence.atlassian.com/display/JIRA063/Adding+a+custom+event

You can expand the notification recipients if you want beyond the single user later perhaps adding a group of users if you wish.
