Missing options for Forms and Advanced Forms creator tools?

Project board type: Jira Work Management

Goal: Create a form that includes checkboxes, radio buttons as well as an option for loading a file.

Problem:

Option #1
Create a form option accessible from the project board:
Limits us to 8 fields
No radio buttons or checkboxes

Option #2
"Advanced Forms" button accessible via Project Settings
Limitless form length
Lots of options (radio buttons and checkboxes)
Missing the ability to add attachments

Question

Is there a way to either:

Bring radio buttons and checkboxes to the original forms?

or 

Bring the ability to add attachments to advanced forms?
