JWM Advanced Forms

Hello, I'm really hoping someone can help me with my question because there is confusing info available. 

I created a TEST JWM project.  On my Test Project I found Advanced Forms, just like Proforma forms on JSM projects - YAY!!!!

I created the 2 forms my client wants but I can't link them to the issue types I have created.  

When I search for info on Advance Forms, it takes me to a 2-sentence posting that they are Proforma forms - but when I click on the link the next page is all about JSM not JWM.  Advanced Forms in Jira Work Management | Jira Work Management Cloud | Atlassian Support

How do I link my advanced forms to the issue types I create?

If you go into your advanced form and then click on the Settings tab you should be able to associate it with the issue type. You should see something like this:
