Form submission not successful

I created a form on Jira Work Management.  

To test it I shared the form link with myself.

I filled in all the necessary fields and clicked on the Submit button.

Result:  I get a message:  Something's gone wrong.

Steps taken to solve the problem:

I tried what is suggested in the message - i.e. Refresh the page and try again

I tried removing some of the fields from the form that I thought might be causing an issue and left only the bare minimum.

Hi, does this happen on issue creation only or on an existing issue? Did you try the same step on a different browser?

Hi Prince, thank you for your answer.  It seems there were required fields missing from the form.  These fields are required by the Default Issue Type Scheme which has been a frustration because you can't seem to remove this scheme from your project.

Anyway,  I added the required fields and now the submission is working.
