Hi,
In my project I have a 3 tiered issue type model - Epic - Task - Sub-task. They follow the same workflow. For some reason, I am unable to create a form on any other issue type than Task. When I click on Create form button it will automatically assume that I want it for Task issue type and even in form creation window, I cannot change the type for which the form needs to be created

Welcome to the Atlassian community.
It appears that Forms in JWM can be used only for issue types at the standard issue level (i.e. Story, Task, Bug) - the level between Epics and Subtasks.
It is understandable that Forms can't be used for Subtasks, because Subtasks must have a parent issue and therefore can be created only from within an already existing issue.
There is a change request to have JWM Forms support the Epic issue type also.
