Unable to create an automated subtask at the end of the workflow

I'm attempting to create an automation that when an issue transitions from one status to another, it creates a subtask. Unfortunately, when the issue transitions, the subtask isn't creating.

In the audit logs, it shows that the automation is running, but it is not producing any sort of error. 

Any guidance on what could possibly going on, would be great.
