What's the error message you got corresponding to the issue collector creation event?

But I'm not getting any error only redirect to the same page again with all the fields filled in with the answers that I already submit with no error message or warning

Hi Hala,

If there is required field for the selected issue type, the same behaviour will be observed. 

Try to check and disable any required field for the selected issue type (Field configurations page).
