Has anyone seen a duplication as a result of their automation rule?

I created an automation rule that works flawlessly when a new sprint is created, a task and subtask is created. However, it started duplicating the task and subtask mid month for no reason and there are no triggers from the rule created?

Hi Alexey,

I can able to create a workflow and i can able to add new status for existing workflows in text mode but I can't able to view a workflow in the diametric mode.

and there is a no workflow session under summary(project setting)
