EXAMPLE :
1) From Not started to pending approval : approvers feild was automatically edited and sent mail to the approvers
2) from pending approval to Revision needed : clear the approvers field and automatically transition back to not started and sent mail to the requestor or reporter,
please any one help me out of this

There must be one on the transition.
Open your workflow and click on the transition and look for a screen that is there.
I'd expect the screen to appear on the "All" -> "Done" transition you have in there.

Thanks for the quick responses! Nothing is clickable on the diagram view of my workflow, so I'm not sure what to click on when you recommend hitting the All -> Done transition.
When I view the 'text' version of the workflow, this is what I see

You need to be in the diagram view of the workflow to work with the "all" transitions.

So when viewing the workflow in diagram view in the top right just above the workflow there should be a Edit button. If you can't see this then you don't have the permissions to modify the workflow.
If you can see it, click it and then click on the transitions and a screen will appear like the one I posted earlier.
