What does the Issue Count in the top left of the Control Chart represent? Regardless of selections of dates, columns, swimlanes, and quick filters I can never get any query of issues with the same selected parameters to match the count displayed in the control chart.

The number in the top left side of the control chart shows the number of issues within the time frame you've chosen (e.g. 3 months) which were worked on and completed during this period. It is natural not to be able to get it from a query, since there is no way to create such a query.
Keep in mind that:
The Control Chart shows the Cycle Time (or Lead Time) for your product, version, or sprint. It takes the time spent by each issue in a particular status (or statuses), and maps it over a specified period of time. The average, rolling average, and standard deviation for this data are shown.
Try to test this chart with an empty project. The issues will appear in the chart once they get in the done status. If you re-open a ticket, it will disappear from the chart. So it will only display done issues, within this time frame you've selected. Issues that have been worked on.
