I'm looking for a way to format a field as a table, like below, and add it to a Jira issue/Initiative.
The first column needs to be text and 2nd columns needs to be filled in by the users, with a number/quantity
I tried to create a custom field but I can't see a table grid format that I can use.
Is there a way to do this. Either with a custom field or another solution (adding a field tab/configure issue screen) ?

Welcome to the Atlassian community.
There is no native "table" type custom field in Jira.
If you have a static set of Roles, you could create a custom field for each role. You could collect these fields on a separate Tab for the screens of that issue type.
Or you could consider adding a third party app that would give you a custom table-type field.
