Is possible to add a "Comments" box to the satisfaction survey?

You can't add any field to the CSAT email that is being send after an issue has been resolved. This predefined email comes with a small section in which the customer can place his experience from the service he/she got. If you thing that this isn't enough, there are apps on the market which could extend this functionality https://marketplace.atlassian.com/search?hosting=cloud&product=jira-service-management&query=survey

The survey email that is sent contains a simple form that allows the user to add a rating and feedback and if you go to Reports > Satisfaction you can see both the rating and the comment.
