Need to create an automation that insert a filed on an issue.

Hi. I am trying to make an automation that insert a fild under certain condition. The client will select a option in the intial request. The automation i create will change de status depending how option is choosed. 
I need automation to insert a different different field for each depending on the option selected.

EX:

1 - Choose yes on request - create a issuel with (On going status) - show Text fild 

2 - Choose no on request - create a issuel with (waiting status) - show URL field

You have to create a form in your JSM project which you will then place on your request type. On that form you will set some section, in which you will state that if yes then show this field. If no, show that field.
