i need certain information from customer when they create an incident ticket. i have a set of question for which i need answer from customer .
I want this question set to be translated into a template which is available to customer during ticket creation.

You could create a form, which you attach to your request type and the customer can fill them in. You can optionally associate the form's field to that of Jira, so you can further work on reports with these fields.
