Great that you found the third party app that you need to remove from your env after my suggestion.

Welcome to the community. In JSM - Request type is based on issue type. Typically, there is not issue dependencies setup.
Issue Dependencies is for Jira Software application projects (to support Advanced Roadmaps for Jira) or Jira Work Management projects. They are not for JSM projects based on my understanding/experiences.
Issue Hierarchy is for Jira Software application projects (to support Advanced Roadmaps for Jira)
So it is not clear on what you are asking for?
Hope this helps.

You need to check your request type configuration (screens) and the issue type screens configuration setup.
What kind of project type is your project?

Hey @Joseph Chung Yin so I have tried editing/configuring the screen schemes but there is no option for "Issue Dependency" and "Issue Hierachy" to remove them?

What kind of project type your are using? Secondly, what you are seeing seems to be not out of the box. Are you also using third party add-ons? If so, you may want to look into those add-ons where those fields may be coming from.
