How can I use post function to attach forms on issue transition?

Currently, we are trying to find a Jira add-on or app that can be a substitute to Jira Automations (see image below). However, we can't find an app that can attach a form on issue transition.

We have tried the following apps and scoured their documentation on anything related to this, however we found nothing:

• Jira Workflow Toolbox
• ScriptRunner
• Power Scripts

Hi @Jira Admin Team , I apologize for asking the obvious, but in your post image the Forms field is blank -- did you choose at least one form and then run the rule? Just want to check, because your automation looks right. If you did specify a form(s) when you actually tried this, would you please post a screenshot of the Rule Details for the first part of the rule (click on the When: Issue Transitions part) so we can see that logic, as well? Thanks.
