I need to add a link that is from a Confluence document (this will work as document template or another form) so the customer can download it, then fill it, and at last attach it in the form from Jira Service Management service portal.
Is that possible to do? Thanks

I don't think it is possible to point the customer to a file directly, but you can link to the Confluence Page where you have the file. You can achieve it by visiting the desired page on the Confluence regular view, getting the page ID from the URL (the ID comes right after "pages/") and appending it to the Service Desk URL:
https://xxxx.atlassian.net/servicedesk/customer/portal/PORTAL_ID/article/PAGE_ID
