Has anyone else had this problem on JIRA Service Management?

Hi @H-Clinic and welcome to the community!

Is it only you you have this problem, or all there are other users as well?

Did you try doing the same action either on incognito mode, or from another browser?

Did you change anything on the create transition on your workflow, like adding a post function? IF the latter fails, then such errors can appear.
