I want to use the automation action "Attach form" to attach a form in the Customer Portal after triggering it.
But I don't know how to attach the form locked for editing by customer.
If this isn't an option, got any ideas?

I may proposed you add-on developed by my team Smart Jira Forms. With this add-on you have possibility to automatically add forms to predefined Jira issues.
