looking for a pre-assessment tool

Hello 

 as part of front door  pre-assessment  projects ,  I would like  to build a  pre assessment tool  where a User would  answer and fill a set of defined Criteria from drop down fields  based on some criteria's  in order to get more visibility on the doability of a project  as well  we need this tool to get as well a visibility to our resources availabilities and their current capacity based on different sprint and projects they work on

To build the pre-assessment tool  I have thought of creating a Jira template with custom fields  , are there other suggestions and need as well to link this template if possible to   capacity planning tools 

are there any adds-on in Jira can fulfill these needs
