I've had AD Integration working previously but for some reason it is now failing, somewhat. I have users loaded from AD in Jira and I can run a test authentication, however my synchronization task is failing. I can't seem to figure out what the issue is. Obviously the server IP is right, the LDAP lookup account is right as I can authenticate. I just can't figure out why the synchronization is failing.

Based on your included error log, I think you are seeing the scenario in the KB LDAP sync fails with error request cancelled.
That KB explains that this is a common error to see when your LDAP filter is not configured in the proper syntax. Try to follow that KB and please let us know if that helps resolve this problem. If it doesn't please let us know more about your LDAP settings in use by JIRA.
