Atlassian/Jira Dashboard Activity Stream failing to auto-refresh

Since about a month the Activity Stream on my Dashboard has been randomly failing to auto-refresh. Sometimes it just goes blank, and other times it has an "Unauthorized 401" error in it. Ctrl+R brings it back to life.

It's not the workflows, it's the workflow scheme.

When you simplify a workflow, it tries to create a new workflow, put it in a new workflow scheme and apply that scheme to the project.  It generates the names for the workflow and the workflow scheme from the name of the project.

But it can't create a new workflow and scheme if the names are already in use.

So, go to admin -> workflow schemes and find the one it is complaining about.  Rename or delete it.  Then try the simplify action again.
