Data in the request form entered first time should get reflected when the user adds in existing form

i have request form in a ticket on jira with set of fields 

first time i will be adding or entering the data in the fields present in the current form

next time when i wanted to add any extra field in the request form the data which is already entered in fields should be reflected along with the new field which will be added second time

can any one give an idea regarding this project

You can't add more of the same field to an issue.  Each field exists once and once only for an issue.

The closest you can get to this is to use the Table Grid Editor app.
