trying to trigger an automation when a specific form is submitted, but for is not showing up in list

I created a simple form that creates a request issue.  I'm trying to setup an action so that when the specific form is submitted, then modify the issue created to add labels.  I used the AI to create the automation rule, problem is that I cannot tie the automation trigger to the specific form.  when I do, the list of specific forms is empty. What am I missing that is preventing my form from showing up in the forms list?

Welcome to Atlassian Community!

The Forms submitted trigger is related to JSM Forms, it cannot be used with forms that you create in Jira, those are different features. JSM Forms are based on an app called ProForma Forms that Atlassian acquired a couple of years ago, while forms in Jira is an in-house feature.
