Hi, if you're an admin, you can revert it back:

Note: Admins can also revert their site to the previous create experience via Settings > System > General Configurations, then set "Simple Create as Default" to off. This applies to the entire site.

@Tomislav Sablic is correct. If you're a Jira administrator and the option is still available on your site, you can disable Simple Create as Default under Settings → System → General configuration to restore the previous create experience for the entire site.

Keep in mind that this is a site-wide setting (not per user), and Atlassian has indicated in their rollout article that the opt-out is temporary as the new create experience becomes the standard.
