in my opinion you cant use a uikit component in an custom ui page. But you can use the components from atlaskit in an custom ui app. The form iis here as example Form - Examples - Components - Atlassian Design System

Hi @Ankit kharola 

It's not possible to use UI Kit components in a Custom UI app, however you could create a UI Kit app, then use the Frame component to build your own custom component to use within it.
